Up to 1,800 patients in Bishopsteignton have been told their GP surgery is 'a thing of the past' and could close within months.
In future a visit to the doctor will mean travelling, something community leaders have said will be difficult for those without cars.
Dr Paul Raby, senior partner with the Channel View Medical Practice, has written to residents explaining why the axe could fall.
He said for 27 years the 'unique charm' of the converted stables had been much loved but the building was no longer fit for purpose and the lease was due to expire.
'Being a converted building has raised challenges for use as a modern medical practice, and despite investing considerable sums of money to bring it up to date, we are well aware of its shortfalls,' he said.
