All three emergency services dealt with one main accident involving six cars and a few minor bumps in the wake of the initial collision just after 9pm at Harcombe Bends.
One female casualty suffered a shoulder injury while another was treated for shock. A male sustained a wrist injury.
Each casualty was taken to a different hospital - Torbay, Royal Devon and Exeter and Plymouth.
The fourth victim, who suffered abdominal pains, did not need to go to hospital.
It’s understood the first accident involved a car rolling over and righting itself after crashing into the central reservation.
The carnage created major traffic disruption with the Splatford Split being closed off by police and diversions set up on the A380.
Two fire crews from Exeter responded to the 999 shout.
A spokesman said ‘multiple occupants’ were treated for shock at the scene.
Inquiries in to the cause of the accident are being pursued by police.
Cars involved in the first collision were a Nissan, Renault Clio, Skoda Octavia, Hyundai, Vauxhall Zafira and a VW Polo.
